OVERVIEW
Tirzepatide is a dual GIP and GLP-1 receptor agonist. Mounjaro is FDA-approved for glycemic control in adults with type 2 diabetes. Zepbound is FDA-approved for chronic weight management in eligible adults and for moderate-to-severe obstructive sleep apnea in adults with obesity.

Pharmacokinetics

Dosing
Label-dependent. Zepbound starts at 2.5 mg subcutaneously once weekly for 4 weeks, then increases in 2.5 mg steps after at least 4 weeks; labeled maintenance doses vary by indication, with a maximum of 15 mg weekly. Follow the current product label.
Monitoring
• Weight and treatment response • Glucose and hypoglycemia risk when used with insulin or insulin secretagogues • Gastrointestinal symptoms and hydration • Gallbladder and pancreatitis symptoms • Renal function when volume depletion is possible • Vision changes in patients with diabetic retinopathy history
Side Effects
Common Zepbound label-reported reactions include nausea, diarrhea, vomiting, constipation, abdominal pain, dyspepsia, injection-site reactions, fatigue, hypersensitivity reactions, eructation, hair loss, and gastroesophageal reflux disease.
Contraindications
• Personal or family history of medullary thyroid carcinoma • Multiple Endocrine Neoplasia syndrome type 2 • Known serious hypersensitivity to tirzepatide or product excipients
